The Grant Per-Diem Transition in Place

Program Summary

The Grant Per-Diem Transition in Place

The Grant Per-Diem Transition in Place program serves individuals in Meridian MS by providing supportive housing and/or supportive services with the goal of helping homeless Veterans achieve residential stability, increase their skill levels and/or income, and obtain greater self-determination. The “Transition in Place” housing model offers residents housing in which support services transition out of the residence over time, rather than the resident. This leaves the resident in place at the residence and not forced to find other housing in 24 months or less. Additionally, supportive services incorporate tactics to increase the Veteran’s income through employment and securing the permanent housing in the Veteran’s name.
